PROGRAM PREREQUISITES

In order to join the STAR Program, you must:

• Be a current and paid DSPN member

• Be a regulated entity in the state of Wisconsin as a licensed AFH or CBRF

• Have a computer/device and Internet access on-site

• Be willing to participate in all components of the STAR Program

 

PROGRAM APPLICATION PROCESS

You need to submit the following to the DSPN office:

• A completed STAR Program application for each participating facility (each licensed facility requires a separate application)

• A copy of your most recent state survey for each participating facility (including a plan of correction if deficiencies were found)

• The STAR Program enrollment fee

  • Adult Family Homes – $25 fee per facility

  • Community Based Residential Facilities – $50 fee per facility

All DSPN members that are approved must agree to the following:

• To be a participant in the STAR Mentoring and Support peer group.

• To participate in quarterly on-line WCCEAL surveys presented by CHSRA for the purposes of data collection.

• To maintain quality standards and quickly report and rectify any deficiencies that may occur throughout the duration of assurance
